About the Role

We launched our 0-25 Disability Service in July 2024 bringing together services supporting children and young people and adult services to deliver better outcomes and promote independence for children, young people, and their families as they prepare and transition to adulthood.  We are currently looking to recruit a full-time social worker for Children’s Social Care element of the 0-25 Disabilities Service, offering you the opportunity to play a key role in achieving positive outcomes for children, young people and families as they transition to adulthood in Lambeth!

As an experienced social worker, you will join a team of 5 social workers led by a Team Manager. Holding a reasonable caseload of statutory and short breaks cases, you will be supported to ensure a high level of performance and provision of a quality service for all our children and young people with complex and severe disabilities, including ensuring a seamless transitioning and preparation to adulthood. The focus of this role will be the delivery of statutory functions aimed at delivering an effective Children’s social care service. You will contribute to connecting with multi-agency teams to deliver holistic services.
